Useful Tips when completing payroll report forms:

  • Follow the detailed instructions attached to your payroll report.
  • For classifications whose description specifically states to include clerical (Incl Cler), add clerical payroll to the regular employee payroll for that classification and enter it in
    column A.
  • Employees that perform multiple duties (clerical and duties described by classification), include payroll under either the classification that closely describes the nature of the business or non-clerical work the employee performs.
  • Individuals or Partners who have previously elected to be covered should be included in the class code that best describes the nature of the business or the type of work the owner performs. Include only the owner who is specifically included. Refer to report instructions or policy endorsement.
  • For individuals or Partners who have previously elected to be covered, the payroll listed on the report should not be more than the maximum or less than the minimum remuneration listed on the back of the payroll report.
