Obtaining a quote

Requesting a quote to apply for workers’ compensation insurance with NYSIF has never been easier thanks to eQUOTE, our state of the art quote system. In most instances, a premium quote will be returned within three business days.

You can apply for your clients by answering the following:

  1. Requested effective date of insurance.
  2. Full name(s) of the employer(s) including any trade name(s) or doing business as name(s)?
  3. Employer(s) main NYS work location, county, and mailing address
  4. Do you have a representative? If yes, name and address?
  5. How long has your company been in business?
  6. Have you ever been insured for workers' compensation?
  7. Your latest experience modification factor and the effective rating date, if known. 
  8. Have you been declined for coverage during the last 12 months?
  9. Have you ever been insured with NYSIF?
  10. Describe your business operations including the products or services sold.
  11. List your estimated annual payroll by type of work and duties for all your employees.
