Print Submit an Application for NYSIF Workers' Compensation Insurance Obtain a NYSIF workers' compensation insurance policy by first requesting a NYSIF workers’ comp quote using NYSIF eQuote. In most instances, a premium quote will be returned within three business days. A NYSIF underwriter may contact you following submission to obtain documents necessary to return an appropriate quote (e.g. loss runs, payroll verification, etc.). When you receive your quote, you can complete, sign and submit an insurance application online. Employers applying for workers’ compensation coverage have the option to pay the deposit premium amount indicated on their NYSIF quote by electronic funds transfer or credit card. All applications must be fully completed and be accompanied by deposit premium payment to be reviewed. If your application is approved, coverage becomes effective at 12:01 a.m., Eastern Time, the day following NYSIF's receipt of your electronically signed application and electronic premium deposit payment. For applications sent via U.S. mail, coverage, if issued, becomes effective the day after the postmark on the envelope containing your signed paper application and deposit payment, or any subsequent date requested.
