Monthly Billing Statement and Information Pages

NYSIF's monthly billing statement is received when premium is due and reflects all transactions that occurred during the billing period. Learn more about our monthly statement at How to Read NYSIF Monthly Billing Statement.

In addition to the monthly billing statement, policyholders also receive various information pages throughout the year. Typical information pages are explained below.

  1. INFORMATION PAGE Received by all policyholders when their policy is written, renewed or revised; The information page summarizes policy coverage and provides details on classification codes, payroll and modifications used by NYSIF to determine premium. There are three types of information pages:

  • Information Page New Policy - received by all new policyholders; summarizes policy coverage and provides details on classification codes, payroll and modifications used by NYSIF to determine premium for the policy period.
  • Information Page Renewal Policy - received by all policyholders approximately 45 days prior to renewal; summarizes policy coverage and provides details on classification codes, payroll and modifications used by NYSIF to determine premium for the policy period.
  • Information Page Revised – typically issued to reflect a change in premium due to a change in classification, experience modification, NYSIF mod, payment plan, payroll and/or period covered; replaces a declaration, renewal or an earlier issued revision for the policy period.

  1. AUDIT Summarizes final premium for a specific policy period based on the actual payroll obtained in a physical examination of payroll records by NYSIF. Open Audit's PDF format to view an example.
  2. DESK AUDIT Summarizes final premium for a specific policy period based on the actual payroll obtained in an examination of the payroll records provided to NYSIF. Open Desk Audit's PDF format to view an example.
  3. REPORT Summarizes final premium for a specific policy period based on a written report of actual payroll submitted by the policyholder. Open Report's PDF format to view an example.
  4. ESTIMATE Summarizes estimated final premium for a specific policy period based on estimated payroll in lieu of actual payroll not made available to NYSIF. Open Estimate's PDF format to view an example.
  5. DEPOSIT RESCIND Sent whenever a revised information page is issued, the deposit rescind gives credit for all deposit premium billed for the period that is being revised. Open Deposit Rescind's PDF format to view an example.
  6. EARNED PREMIUM RESCIND An itemized transaction that reverses all previously issued earned premium for a policy period. Open Earned Premium Rescind's PDF format to view an example.
  7. DIVIDEND RESCIND An itemized transaction that reverses all previously issued dividends for a policy period. Open Dividend Rescind's PDF format to view an example.
  8. AUDIT ADJUSTMENT An itemized transaction that adjusts the audit bill(s) in a policy period, either to correct the amount of payroll billed, to reallocate payroll from one classification to another, or to delete payroll billed for a subcontractor when proof is received that the subcontractor has his own coverage.   9. DIVIDEND An itemized transaction that credits the audited premium of each member of a safety group by a predetermined percentage (called a dividend) declared by that safety group. Click to Open
  10. RECOMP An announcement to add, change, or delete an Experience or PAP modification. Open Recomp's PDF format to view an example.
  11. ADJUSTMENT EXP/PAP MOD An itemized transaction automatically issued after a recomp; this transaction corrects the experience modification/PAP applied on all audit bills produced for the policy period covered by the recomp. Open Adjustment EXP/PAP/MOD's PDF format to view an example.
