NYSIF eFROI® - Electronic First Report of Injury

Once you have determined when to file a report of injury or illness, early filing can help lower claim costs by allowing NYSIF to actively manage the claim as soon as possible. Using NYSIF eFROI®, employers can electronically file Form C-2 "Employer's Report of Work-Related Injury/Illness" with NYSIF and the WCB 24/7, eliminating the need to mail a paper C-2.

Note: Statements made in Form C-2 can be held binding by the WCB. Contact NYSIF for language guidance in reporting questionable claims. Immediately notify NYSIF of any new developments/information regarding a case after filing Form C-2.

NYSIF eFROI® retains a permanent report of injury, retrievable from our website with the claim number, policy number and last four digits of the claimant's social security number. Policyholders who do not want to report electronically can download Form C-2 for paper filing.

Additional Responsibilities - Forms C-240 and C-11

Promptly provide the NYSIF case manager accurate wage and attendance information for the injured worker by submitting WCB Form C-240, “Employer's Statement of Wage Earnings Preceding Accident,” to help establish the validity of a claim and compensation rate, if awarded.

Identify light duty or modified job assignments to facilitate the injured worker’s return to work. When there is a change in work status use WCB Form C-11, “Employer's Report of Injured Employee's Change in Employment Status Resulting From Injury,” and notify your NYSIF case manager.
