First Report of Injury (FROI)

NYSIF workers’ compensation policyholders can electronically submit a workplace injury report to NYSIF any time with NYSIF eFROI®, the fastest way to report workplace injuries to NYSIF. This eliminates delays, reduces errors in reporting and allows NYSIF to control claims costs from day one.

Not all accidents result in compensable claims. However, an employer may report an employee injury to NYSIF for reporting purposes only.  NYSIF eFROI® creates a preliminary record of injury so we can begin an accident investigation and proactively manage the case.

When making a claim, it is the employer’s responsibility to file Form C-2, "Employer’s Report of Work-Related Accident/ Occupational Disease," with the Workers’ Compensation Board (WCB). Upon request, NYSIF will arrange with the employer to submit a C-2 to the WCB on the employer’s behalf.

Employers may elect to submit a First Report of Injury by telephone at 1-877-INS-FUND (467-3863). A NYSIF representative will help the caller complete a C-2 form to file with the WCB if a claim is made.
