Tips for Filing C-2s

Use careful language when reporting an accident. For questionable claims, preface your statements with, "It is alleged that…" or "The employee claims that..." Statements made in Form C-2 have been held to be binding by administrative law judges. Notify NYSIF immediately if you are suspicious about a claim.

Meet all filing deadlines and maintain adequate records for at least 18 years. Failure to do so is a misdemeanor that could cost you up to $3,500 in fines and penalties.
