Amending a Policy

A NYSIF policyholder or the policyholder’s authorized representative should request to amend a policy whenever there is a change to any of the following:

  • Insured’s address;
  • Insured’s name;
  • Ownership of an insured;
  • Telephone number;
  • Operations or nature of business;
  • Payrolls (more than 10% change);
  • Additional employer;
  • Change in location;
  • Representative.

Requests to amend a policy should be made in writing, by regular mail, fax, or email to the underwriter. In some instances, the change can be requested by telephone, but usually will require written confirmation.
