All requests to amend a policy must be made in writing to your underwriter by email, fax or postal service whenever there is a change to any of the following:
- Insured’s name
- Insured’s address
- Insured’s ownership
- Insured’s telephone number
- Operations or nature of business
- Payrolls (more than 10% change)
- Additional employer
- Insured’s location
- Insured’s representative
